New Patients can register if they live within our Practice Area
A Registration Form will need to be completed and this is available from receptions at both Builth Surgery and Llanwrtyd Branch Surgery.
It is advisable to register at Llanwrtyd Branch Surgery if you live in or towards Beulah or Llanwrtyd Wells.
At registration you will be allocated a treating doctor. It is important that you are aware when this doctor is available for consultations. The doctors run open access surgeries. Patients can express a preference of treating doctor and this can be made to reception.
New Patient Medical
It is practice policy that all new patients (over the age of five years) have a nurse consultation to check on their past and current state of health. Our nurse see patients by appointment. Please make an appointment when registering.
Before you attend your medical, you will need to complete an information form for each person (including children) who will attend. The results will be included in your medical notes.
Please bring specific evidence of your current medication and a urine sample to your new patient medical appointment.
Print a copy of the Medical Questionnaire (Word 39Kb) and bring it to your nurse appointment
